Description

This toolkit is a guide for local program planning in support of the Ontario Public Health Standards (OPHS). The framework guides the assessment, planning and implementation, and evaluation of public health programs.

Steps for Using Method/Tool

The toolkit guides users through four program planning steps:

  1. Assessment – Define the problem.
  2. Plan – Create a plan to complete deliverables.
  3. Implement – Act on the evidence delivering high-quality programs.
  4. Evaluate – Assess the effectiveness of your implementation efforts.

Each step is complemented by tracking forms, worksheets and resources.
