Description

The Program Sustainability Assessment Tool is a self-assessment tool for evaluating the sustainability of a program at the community, province/state and national levels. This tool also serves as an aid for sustainability planning.

Steps for Using Method/Tool

The tool provides 40 questions using a Likert scale. It addresses the following eight domains:

  1. Environmental support
  2. Funding stability
  3. Partnerships
  4. Organizational capacity
  5. Program evaluation
  6. Program adaption
  7. Communications
  8. Strategic planning

The final scores conceptualize the program across each domain, highlighting which area users may want to focus their efforts on to increase sustainability capacity.
